Participant Waiver
& Release Form

ASSUMPTION OF RISK

I, the undersigned participant, acknowledge and fully understand that participation in basketball and related court activities involves certain inherent risks, including but not limited to: physical injury, sprain, fracture, concussion, cardiovascular events, or other bodily harm. I voluntarily assume all risks associated with participation in this event.

RELEASE OF LIABILITY

In consideration of being permitted to participate in this NBA court event, I hereby release, waive, discharge, and covenant not to sue G300 DreamLegacy Event, its organizers, owners, facility operators, employees, volunteers, sponsors, and agents (collectively, "Released Parties") from any and all liability, claims, demands, actions, and causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained by me during or in connection with this event, whether caused by the negligence of the Released Parties or otherwise.

INDEMNIFICATION

I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or cost they may incur due to my participation in this event, whether caused by my negligence or otherwise.

MEDICAL AUTHORIZATION

I authorize the Released Parties to seek emergency medical treatment on my behalf if I am unable to provide consent. I agree to be responsible for all medical costs associated with such treatment. I acknowledge that the event organizers are not liable for any medical decisions made on my behalf in good faith during an emergency.

MEDIA RELEASE

I grant permission to the Released Parties to use photographs, video recordings, and any other media captured during the event for promotional, marketing, or informational purposes without compensation to me.

RULES & CONDUCT

1. I agree to follow all rules and regulations of the facility and the event.
2. I agree to wear appropriate athletic footwear and attire at all times on the court.
3. I will not participate while under the influence of alcohol, drugs, or any impairing substances.
4. I understand that unsportsmanlike conduct or dangerous behavior may result in immediate removal from the event without refund.
5. I will respect all equipment, facilities, staff, and fellow participants.
6. I acknowledge that participation is at my own risk and that I am in adequate physical condition to participate.

GOVERNING LAW

This agreement shall be governed by and construed in accordance with applicable state law. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

ACKNOWLEDGMENT

I have read this entire agreement, I understand it fully, and I am signing it voluntarily. I am 18 years of age or older, or if under 18, my parent or legal guardian has signed on my behalf.
